Default should this be welded in?

while taking out one of the rear seat belt bolts, this bolt and this piece started spinning! then i stripped the bolt head so i had to cut the head off. then this fell to the floor. should it had been welded and the torque wrench broke the welds or does it just cinch up when tightened? if so, why didnt any of the other 3 do this? needless to say, if anyone has on i could buy, i'd appreciate it. not worth the trouble tapping the bolt.

Those are welded to the floor......I don't know if I have a spare...will have to look.

New floor, drill a hole to bolt it down. Then take it off, drill two more holes on either side, 1/2 inch holes. Bolt the piece back up ( after its all blasted and bare metal and Mig weld those outside holes up.
I have done this many times
