Does anyone have a copy of this form they can make a high resolution scan of or .pdf?

I think there are two or three pages, I have lost mine.

Anyone who has a modern day PHS packet for a 69 should have the copies.

I will scan it tomorrow in a very nice .pdf file and have it loaded here and on my web site.

I have PHS docs on my cars but those order docs are interesting. Searched and found one posted by Diego but that was for 68 Big Pontiacs. Love to see a 68 A-Body one! Anyone direct me to one?

That is my website. Hours and hours of scanning and Photoshopping.
Steve, can I add your '69 A-body order form to my site? It would be nice to have the top portion that seems to be missing.

Bill absolutely add it; however that is the best scan I can get as it is legal sheet and impossible to scan the full thing.

Might add that RAIV rocker deal too if he give us permission

My scanner does 11x17.

And, I've got the 69-I-62 sheet in a .pdf form.

492 is Deck Release.

I would assume just like 381 is for Power Antenna, tricky listing of codes by Pontiac.

So what is 494?

Sorry I missed that 494 is rally stripe. This is on my ram air 3 car that was ordered with the judge stripe but not a real judge. It is listed on the build sheet.
