Signal-Stat Flare-Stat 105 hazard flasher - What is correct for 1965 / Installation

Hi,

I would like to purchase and install a Signal-Stat Flare-Stat 105 hazard flasher.

However, there is a confusing variety of versions. I would be happy to see pictures of the correct one and the installation instructions as well.
