I would imagine (rather than measuring it out on every vehicle) they had some kind of fixture or template to help locate the decal. The only problem with that is they typically are clunky and slow you down, so they fall into disuse as soon as management is not looking. The guy eyeballs it and this is what you get.

These days we build some locating features into the premask to help place the part. Remove the premask and the graphic should be in the right spot. The tradeoff is the more extra material you add the bigger the part gets and the more it costs.

Also - probably a different guy putting the LH side on vs the RH side.
