I just saw this thread and scanned through it but I didn't see any mention of the newer 'anterior approach'. If I missed it, sorry for the repeat info. Using an anterior approach the docs don’t have to cut away and through muscle compared to the more traditional surgery. They actually get up and on your feet the same day. It’s really remarkable.

Dr Berry was the first doc in the area to train for this and that was a few years ago and since then he has trained several other docs. Here is a link to an interview done recently discussing the anterior approach.
