I needed those adapters when I was running a third gen steering box and they dropped in. Relative to the pitman arm issue, I have always separated the pitman from the drag link first then pulled the pitman off prior to removing the box. I then install the box, then the pitman, then reattach the drag link.
You may have to separate drivers side tie rod as well but I don't remember as I have a puller that makes quick work of all of the tapered joint stuff.
