There is at least one matching that description , and i think they have laid claim that it is the only one with that exact combo known.
(IV 4spd OO vinyl top)

Fully restored and was for sale - for a while - 2 or 3 years back.
Was on eBay a couple or few times

It was Non-AC , btw
