Default Changing motor mounts

I have a 68 with a 350 A/C and Power Steering...While changing plugs which is terrible on the passenger side , I noticed that the motor is tilted slightly toward the passenger side. It appears as if one of the motor mounts has settled/worn out or craped out. Is this common? and what would be the process to change them. Can it be done one side at the time with the motor in the car? If so can someone provide some details on how it is done??? I was thinking that would be a great time to slide some headers or Long branch in...any advise on what is the better option when it comes to fit/ground clearance and price ect...

The engine is supposed to sit slightly cocked to the passenger side. This is totally normal. Not exactly sure the reasoning behind it. I've heard everything from torque of the engine to providing adequate room for the engine. Maybe someone else can chime in as to an explaination why.
